• NimCore / Hormone Optimization subscriptions

Scheduling

When it is time for your annual follow-up, you will receive communication with instructions on how to schedule. Please complete it promptly to maintain continuity of care.

Purpose

The annual telehealth visit allows your provider to:

• Review your progress and treatment history
• Assess whether any adjustments are needed
• Address any questions or concerns you may have
• Ensure your treatment plan remains appropriate
